The Patient Portal is a secure website that allows you to communicate with your doctor's office and to view parts of your personal health record. This portal will help you better manage your care and enhance your partnership with your health care team.You can track your health, request prescription renewals, request appointments,view lab results, ask questions, and much more.
In addition, patients have access to NorthShoreConnect, a secure, free, online portal that allows diabetes patients to upload health and glucose-monitoring information and send it directly to their physician from the comfort of home or their mobile device. NorthShoreConnect also lets patients view test results, schedule appointments, renew prescriptions and view and pay medical bills in one convenient location.
The team at the Diabetes Education Center includes board certified endocrinologists, registered nurses and registered dietitians who have national recogni-tion though the ADA. These Cert ified Diabetes Educators (CDE) are specialists in teaching you to better manage your diabetes.
To see your labs, click on the lab/diagnostic imaging tab. Your labs will be listed individually by date. Just click on the name of the lab to see the value, a normal range and any notes your physician may have written.

Your test results will be made available to you through the patient portal soon after they are finalized, and likely before your provider has had a chance to review them. It can be difficult to understand the meaning of test results without the input of your provider.
