What is the patient portal and how do I use it?

Internal Medicine Associates recommends all patients utilize our Patient Portal. You will be able to verify appointments, send non-urgent messages and questions to your physician and retrieve test results through our patient portal.

How do I sign up for access to the MyChart patient portal?

To sign-up for access, visit the MyChart patient portal. You'll need the activation code located on your billing statement or your after-visit summary. This code will enable you to log in and create your own username and password. If you do not have an activation code, you can fill out an activation code request form.

How do I get a same-day diagnosis from UChicago Medicine?

Complete an online questionnaire (e-visit) through your MyChart account to get a same-day diagnosis and a treatment plan from UChicago Medicine clinicians for common conditions. The Express Expert Cancer Opinion program provides newly diagnosed patients a 15-minute introductory virtual session with a world-renowned cancer expert at no cost.

How do I Reset My Password for my Patient Portal account?

On the portal log-in page, enter your username and password. Select Log on. The Password Recovery Security Question Setup dialog box opens. Follow the instructions to set up the security questions for your patient portal account. If you forget your password, you can reset it yourself by correctly answering your security questions.
