What is navigating care's patient portal?

We have partnered with Navigating Care's patient portal to offer a secure online website that gives you, as a patient convenient 24-hour access to your personal health information and medical records from anywhere. You may also designate a personal representative to access to this information electronically on your behalf.

What's new with NHOH's EMR?

New or returning patient? Signup or Sign In NHOH is transitioning to a new EMR (electronic medical record) in February, 2018. This transition will include a new patient portal as part of their system.

What is the patient portal and how does it work?

The patient portal is a great way to obtain lab results, review your appointments, and communicate with your physician. Please provide your email address when checking in and you will receive an invitation to sign up for the portal.
