Patient Forms

You can bring completed patient forms to your first visit. You can also enter or update your contact information, medication list, allergies, past medical history, and pharmacy information through the Patient Portal. You may need to ask for your user name and password from our office before you can access the Patient Portal.

About Appointment

Please be on time for your appointment and arrive 15 minutes early if you will be completing forms in the office. We reserve the right of charging a $50 fee for appointments cancelled within less than 24 hours of the scheduled date and time.

What is an FNP?

FNPs are certified by the Accreditation Board of Specialty Nursing Certification (ABS NC) and the National Commission for Certifying Agencies (NCCA). Each physician assistant or nurse practitioner is under the supervision of a physician who, along with a medical assistant, work as a team to provide optimum patient care.

Can you enter the office without a mask?

Cloth or surgical masks must be worn during visits to our offices. Masks should be clean and cover the mouth and nose. Entry will not be granted without a mask.

What is medical dermatology?

Is cosmetic surgery covered by insurance?

For any patient receiving cosmetic treatments or treatments not covered by insurance, the estimated cost of treatments will be discussed prior to undergoing procedure. Unfortunately, no assurances or guarantees can be given regarding an individual’s response to treatment for cosmetic procedures.
