PHN is a small group of Advocates dedicated to helping individuals enroll into Prescription Assistance Programs.
Prescription Assistance Programs are put in place by drug manufacturers and allow patients who qualify to receive brand name medications at little or no cost. The problem with these programs is that the enrollment process can be confusing and time consuming.
A: The Med List helps reduce medicine errors by promoting communication between patients and their caregivers. Patients keep the list up-to-date by presenting it to their doctors and hospital caregivers at each visit. The healthcare staff will add the new information to your medical record to make sure your medicine list is up to date.
Always keep the Med List with you so it would be available in an emergency or a routine doctor visit. Many people take several medicines daily and it can be very difficult to remember the medicine name and dose. Click here to download a medication list form.
Ascension Medical Group is partnering with athenahealth, a best-in-class provider of cloud-based physician practice management and patient communications software. This will improve the way we manage our physician practices in everything from appointment scheduling to billing and payment processing. While we're rolling this upgrade out across our clinics, please select your portal based on where you received services.
A few days before your appointment, you will receive a text message or email from Phreesia (the check-in platform). Click the link provided and follow the simple on-screen prompts. From here, you can pay your copay, update your medical information, or provide important information. When you arrive for your appointment, simply let our front desk team know you have already checked in. At that time, you can also ask for help with using the mobile check-in platform.

Once you’re registered, you can sign in to MyChart from your home computer at MyChart.childrenswi.org. You also can download the MyChart app free from your phone’s app store by searching “MyChart.” Select Children’s Wisconsin from the list to sign in to your account.
MyChart offers secure online access to portions of your child’s medical record. It enables you to easily and securely manage and receive information about your child’s health.
Once you have the activation code, you have 60 days to go to mychart.childrenswi.org and activate your account. We have staff at the Family Resource Center who also can help you create your account.
