MyChart will automatically remove any adult proxy at midnight of the child’s 12th birthday. There are exceptions to the rule, such as adolescent patients that are developmentally impaired or differently-abled, or an emancipated teen.

Neighborhood Health offers primary medical care services including Family Medicine, Internal Medicine, Pediatrics, basic Gynecology as well as onsite laboratory services in most of our clinics. We provide regular physicals, well-child visits, urgent/sick visits, chronic disease management, women’s health, and preventive services such as vaccines and cancer screenings. We provide referrals for services that we do not offer including specialty care. We also offer medical care for patients with HIV. We do not offer obstetric services.

When you call 1-833-867-4642 you will get a member of our skilled and compassionate Patient Navigation Center team who can assist you with navigating your appointment scheduling needs, connect you to important clinical, dental, and specialty services, and answer any medical records or referrals related questions you may have. When you call Neighborhood Healthcare, our Patient Navigation Center is your first point of contact.

The goal of the Neighborhood Family Clinics is to provide quality medical care at a reduced rate. Eliminating the administrative expenses associated with a visit to a doctor allows them to pass along significant savings. These savings will be experienced by both the City and by covered members.

Generally you will not receive any statement of services that will be paid for by the City. At the time of service if you need a statement or a receipt of some kind (for example: your child’s coach wants confirmation they had their physical) you should ask for it when leaving the Clinic.
Orthotics (will be covered when medically necessary, please see the City’s Master Plan document for specific medical conditions for which this service is commonly paid)
To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.
All messages sent to your provider’s office are transferred via a secure server compliant with HIPAA standards.
The RWJBarnabas Health Patient Portal is a free service that provides convenient access to your personal health care information in a secure online environment. With the patient portal, you can view select test results, access your medical records and track your health information.
At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records. Due to the sensitive nature of radiology and pathology test results, these are not available on the patient portal and must be obtained through the hospital’s Health Information Department.
RWJBarnabas Health is in the process transitioning all of our hospitals to a centralized patient portal called MyChart where all hospitals’ records will eventually be contained within one system for your convenience. Please be assured that you will always have access to your medical records.
