New patients to Wake Nephrology Associates are scheduled by physician referral. Our New Patient Referral Form is located under FORMS . Once we receive the New Patient Referral Form, our office will contact the patient directly to schedule the new patient appointment.
New patient materials including Patient Registration Form, Notice of Privacy Practices, Consent Signature, and Health History form. We will mail these to you in advance of your appointment, or they can be printed directly off our web site from the FORMs tab. Please complete these in advance or arrive 15 minutes early to do so.
Current medication bottles (including all prescriptions and over-the-counter medications). Insurance cards. Photo ID. Co-pay, if applicable.
If you are unable to keep your appointment, please give our office a minimum of 24 hours’ notice. If 24 hours’ notice is not given, a $25.00 no show fee will be charged for missed appointments.
Our providers are available 24 hours a day, seven days a week, including holidays.
