If an individual tests positive, PPH C nurse case managers will continue to monitor the individual while in isolation. If it is determined that further healthcare is necessary, PPHC will help coordinate care for the individual. Get more information about quarantine and isolation.
Dedicated nurse case managers from the Protect Purdue Health Center will provide guidance and help coordinate next steps for individuals who test positive for COVID-19. Positive individuals will be required to self-isolate and be on campus or participate in public activity for any reason until they receive medical clearance from the Protect Purdue Health Center. Contact tracers will identify and, when necessary, contact individuals who have been in high-risk close contact with someone who has tested positive. Academic case managers will be assigned to students who test positive to help ensure they don’t fall behind academically.

myPurduePlan is a web-based tool for students and their advisors to monitor academic progress toward degree completion. myPurduePlan also allows students and advisors to plan future academic coursework.
The myPurduePlan worksheet shows a review of past, current and "planned" coursework and also provides information regarding completed and outstanding requirements necessary to complete a degree/major/minor/concentration. The worksheet also contains an Academic History link that allows the student to review course work completed by term.

The Patient Portal is an online service that provides patients secure access to their health information. Various features may be available on the portal at your practice's discretion, including the ability to send messages to your health care providers, schedule appointments, and pay bills online. top
If you’re having issues logging into your patient portal, please contact your medical provider’s office for support.
Click the Billing tab. Your recent charges will appear listed under Recent Charges Payable Online.
Click the Sign Out link at the top right of the screen. Alternatively, if your keyboard remains idle for 10 minutes or more, you will receive a pop-up window asking if you are still actively using the portal. It is at your provider's discretion to make test results available. Your provider must authorize the release of your test results in order for them to post to your Patient Portal account. Only test results which are considered appropriate for release will be accessible through the Patient Portal. top
MyPurdue Proxy: A student can give parents, legal guardians, or other trusted parties access to view certain pieces of their student information online.
What is a passphrase? A passphrase is a unique code word or phrase that may be assigned by a student to each proxy he or she sets up.
If a proxy would like to update their email address, they need to log in to their proxy account with the current login information, enter their new email address into the email address field and then click "Save." The proxy will then need to use that updated email address to log into their account in the future.
A student can modify the proxy access by checking or unchecking any of the boxes in the "Authorization" tab. If you want to add access to a page, check the box. If you want to remove access to a page, uncheck the box.
Lastly, if the proxy has not viewed any information, the student will have access to the button "Delete Proxy Relationship.". The student can click that button if they desire. Note: This button disappears once the proxy has a viewing history.
Yes. Students can see how often a proxy has viewed a given information page by accessing the "Authorization" log. To access this log, log in to the myPurdue portal, click on "Proxy Management," and then click on the proxy’s name to expand their profile. Next, click on the "History" tab.
If the student has removed your access to that information page, it will no longer appear in the list. Alternatively, students set the start and stop dates for proxy access when they set up the account. If the stop date is reached, all access will be removed.
