How do I reactivate my MyChart account?

To reactivate your account, please contact the MyChart Customer Service Center 8 a.m. to 4:30 p.m. weekdays: submit an online form. After verifying your identity, we will give you a temporary password to use to login; you'll then be asked to create a new password.

How long does my ochsner activation code last?

For your security, your activation code expires after 45 days and is no longer valid once you use it. You can now sign up for MyOchsner directly by clicking the Sign Up Now button on the sign-in page, and then the Sign Up Online button. Once you fill out and submit the form, your MyOchsner account will be created.
