Proxy Access - Birth parents or legal guardians may request proxy access to a minor’s (under the age of 18) MyLVHN account by completing the Proxy Access Authorization Form at your child’s provider’s office.
Lehigh Valley Health Network (LVHN) takes very seriously its obligation to protect the confidentiality of your personal information. Our mobile application for patients, MyLVHN for iOS and Android, is intended to connect to servers and systems operated and maintained by LVHN in order to provide you secure, mobile access to those systems and to your health information.

To share information with the LVHN Patient Portal App, you will need to install the app and look for your primary healthcare organization.
TO ENROLL AT YOUR DOCTOR'S OFFICE: 1 Complete adult or child LRH Physician Practice Portal Registration form at your doctor's office, or print form from the Patient Portal on the LRH website and mail to Patient Portal Helpdesk (address on form). 2 Once we receive your registration form, your account will be web-enabled. We will email you a temporary password. 3 Go to the Physician Practice Patient Portal and login using temporary password. 4 You will be asked to validate your identity by entering your date of birth or phone number. 5 You will be asked to reset your password and select a security question. 6 You will be asked to read eClinicalWorks consent form. Once complete, click Next. 7 You will be asked to read the Practice Consent Form. Check the box to agree to terms of use and click Agree.
Lakeland Regional Health Medical Center Patient Portal is a secure online service that provides you with access to healthcare information contained in your global health record. It also allows you to safely and quickly share important and possibly lifesaving electronic medical information with healthcare facilities and providers.
A deductible is the initial amount of "covered" health costs that you pay before your insurance plan begins reimbursement. A deductible is usually a set dollar amount such as $250 or $500.
If your insurance pays all but a deductible or co-payment, you will be required to pay an estimate of your portion of the bill at the time of service. The hospital will file the claim for you. After insurance has made payment, you will receive a bill asking for payment on the balance using cash, check, or credit card.
