How do you set up a patient portal?

With My Seton Health, you have access to:

  • A Summary of treatment during your hospital stay

What is FollowMyHealth patient portal?

Secure internet sites (patient portals) managed by your health plan, hospitals, or other healthcare professionals are another tool for organizing and storing medical records. A patient portal is a secure website through your doctor’s office, hospital, or other health systems that allows you to view your health and medical information.

Can I link my health information to a health app?

If you would like to access your health information via a health application of your choice, please call 866-465-9005.

What if a provider I want to send a message to is not listed in the drop-down box?

Not all providers will be able to receive a message through secure messaging. If the provider you are trying to send a message to is not listed, please call your provider’s office.

Can I make a clinic appointment online?

Most routine clinic appointments can be scheduled through MyHealth Info. If you have special circumstances that require more care coordination, please call your clinic directly.

Can I pay my bill online?

Yes, once inside MyHealth Info, click on the Billing button. Click on the account you wish to pay. Then click on the Make a Payment button to the right of your bill. If you have both hospital and clinic bills, you will need to pay each separately.

Where does my health information in MyHealth Info come from?

All of the information comes from your Northfield Hospital + Clinics electronic health record. This ensures that you have access to the most accurate, up-to-date information possible.

I forgot my password. How do I get a new one?

You can get a new password by clicking on the “Forgot password” link on the MyHealth Info welcome page. You will be prompted to enter your Logon ID and email address. You will receive an email with instructions to reset your password.

I forgot my Login ID. How Do I get a new one?

If you forget your Logon ID, please contact MyHealth Info Services Monday through Friday 8 a.m. to 4 p.m. at 866-465-9005 or via email myhealthinfo@northfieldhospital.org (link sends e-mail).

How to Enroll

Not Enrolled? Contact your doctor's office directly for information on how to request access. Please be prepared to provide your email address. NOTE: While offices transition from paper charts to an electronic health record, not all of your information may be immediately available.

Connect Your Third-Party App

Munson Healthcare now offers you the ability to securely connect your health records to some third-party websites, health management apps or diet/fitness trackers like Apple Health. Apple Health allows you to view your health records on you Apple Watch, iPhone, and iPod.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Physicians Regional Healthcare System will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.

Note to previous Portal Users

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.
