There are two ways you can sign up for your MyHealth@Memorial patient portal account:
1. Receive invitation email from FollowMyHealth. You will be sent an invitation to the email address you provided at registration or when you presented your ID. The email will come from FollowMyHealth with the subject line of Invitation to join MyHealth@Memorial.
After your account has been created, go to MyHealthatMemorial.com to sign in or click the Log In Now button below.
