This e-mail will include your log-on which is your e-mail address, and a link to verify your last name and date of birth.

How to update my information on Riverchase?

To update your personal information in the Patient Portal: 1. Click "My Health" at the top of the screen. 2. Navigate through the different areas to update both personal and medical information. Some areas of information are not editable. Contact your local Riverchase Office for questions regarding this information.

How many characters are required for a password?
