What is UnityPoint portal?

The MyUnityPoint patient portal is a tool to help you manage your health and communicate with your providers. It does not reproduce a patient's entire medical record online, but instead provides access to selected information such as prescriptions, basic laboratory results, summary medical history, and appointments. UnityPoint Health reserves the right to limit health information in the patient portal on a case-by-case basis. For a copy of your full medical record, please contact the Health Information Management department of your hospital or the clinic administrator of your local clinic.

Of the information we learn about you from your visit to the MyUnityPoint patient portal, we store the following: the IP address from which you access the Internet along with the date and time that you access the portal. This information is used to measure the number of visitors to the various sections of our site and identify system performance or problem areas. We also use this information to help us improve features of the site and to make the site more useful.

What is the purpose of UnityPoint?

The purpose of this portal is to provide our patients with direct access to information about their health care treatment at UnityPoint Health and to communicate with their UnityPoint Health providers . As we provide you with health care, we maintain records that include information such as your medical history, current conditions, your treatment plan and treatments provided by our health care team, including therapies, surgeries, medications and lab test results. The MyUnityPoint patient portal powered by MyChart is an internet application that enables a patient to have secure web-based access to selected medical information about the patient.

Can you change your password on MyUnityPoint?

For this reason, it is important that you keep the information you use to log into the MyUnityPoint patient portal completely confidential. You should prevent disclosure of your activation code, ID or password, and you should change your password if you believe that your security may have been compromised. In addition, you should change your password regularly to help maintain the security of your account. You can change your password online at any time by logging into MyUnityPoint and choosing the "Change Password" option.

Does UnityPoint have a secure connection?

All communications between you and your UnityPoint Health provider using MyUnityPoint are carried over a secure, encrypted connection. However, any person with access to the e-mail account to which such messages are sent will be able to see the notifications.

How long does it take to sign up for UnityPoint?

Anyone 18 years of age or older and is a patient of UnityPoint Health can register to use MyUnityPoint. It just takes five minutes to sign up, and your account will start within 1 to 3 days.

Can you use my unitypoint for emergency?

Remember, MyUnityPoint should not be used for urgent needs; for medical emergencies, call 911.

How to Register for MyUnityPoint

If you don't already have access to our patient portal, register for MyUnityPoint by visiting chart.myunitypoint.org. Request an activation code and it will be sent to your email. Authorization Form

An authorization must be completed in its entirety to receive a copy of your medical records. Please mail the authorization back to the HIM Department at the appropriate address listed above. There may be a fee based on the type of request. Media formats available include paper, CD and DVD. Postage fees will apply if the request needs to be mailed.

Request for Birth Certificates

If you need your birth certificate, please contact the Iowa Department of Public Health, as we do not have copies of birth certificates.
