MyChart is the application UnityPoint Health uses to power its MyUnityPoint patient portal. You may come across the MyChart name from time to time when registering for access or using a mobile device to access records and health management tools. How do I sign up for a MyUnityPoint account? 1.
The MyUnityPoint patient portal is a tool to help you manage your health and communicate with your providers. It does not reproduce a patient's entire medical record online, but instead provides access to selected information such as prescriptions, basic laboratory results, summary medical history, and appointments.
In order to grant you a MyChart account, we will verify your identity using questions from a third-party verification system. Once verified, you will be able to create your MyChart account. If you have any questions, please contact your clinic.
If you don't already have access to our patient portal, register for MyUnityPoint by visiting chart.myunitypoint.org. Request an activation code and it will be sent to your email. It may take up to a week for you to receive the code.
