If I cannot log in to my MyChart account after trying a few times, what should I do? To protect your health information, your MyChart account will lock after five incorrect login attempts. Please call the MyChart help line at (312) 563-6600 630-724-5080 between 8 a.m. and 6 p.m., Monday through Friday. A help line representative will unlock your account and show you how to reset your password.

Your activation code will be printed on your after-visit summary, can be printed for you or sent to the email address we have on file for you, upon your request. You will not need to use this code after you have completed the sign-up process.

How do I find out what it is? Click on the "Forgot MyChart Username?" link on the MyChart login page. When prompted, enter your name, the last four digits of your Social Security Number, and your date of birth to receive your Username by e-mail.
If you forget your password, you will need to verify your username, date of birth and zip code. If you do not know any of the information below or you are having trouble recovering your password, contact the MyChart helpdesk (503) 494-5252.
To sign up for your own MyChart account, you will need an activation code. This code is located either on your After Visit Summary or Discharge Summary, depending on whether you were seen in a clinic or were admitted to the hospital.
