Visit MyChart and click Sign Up Now to request an online activation code. You also may be issued an activation code during your clinic visit. For your security, your activation code expires after 45 days and is no longer valid after the first time you use it. We take great care to ensure your health information is kept private and secure.

What is the epic MyChart patient portal?

We have partnered with LCMC Health for use of their electronic medical record known as EPIC. With the EPIC MyChart Patient Portal, your doctors are at your fingertips. If you have an existing MyChart account with LCMC Health, you do not need to create a new account.
