If I cannot log in to my MyChart account after trying a few times, what should I do? To protect your health information, your MyChart account will lock after five incorrect login attempts. Please call the MyChart help line at (312) 563-6600 630-724-5080 between 8 a.m. and 6 p.m., Monday through Friday. A help line representative will unlock your account and show you how to reset your password.

Your MyChart gives you a way to invite caregivers to see your health records. This can include upcoming medical appointments, test results or recommendations for treatment.
To access eCheck-in from the MyChart website, click the Visits tab and then select Appointments and Visits. Once on the Appointments and Visits page, click eCheck-in.
To review charges and make payments wherever you are, log into your MyChart account and select “Account summary” from the “Billing” tab dropdown in the main menu. You can also access financial assistance and insurance information from the “Billing” tab.
To review your medical history, log into your MyChart account and navigate to “Medical History” under the “Health” tab
MyChart helps you schedule an appointment for the next available time slot or add yourself to the list of expected patients with just a few clicks.
Access the Goals tool by logging into your MyChart account and navigating to your “Plan of Care” under the “Health” tab. If you don’t have any goals set, talk to your provider about leveraging this powerful tool for your health.
To sign up for MyChart, you will need an activation code. This code allows you to log on and create your new MyChart user ID and password, which you will then use to log on to MyChart. All patients over age 18 will receive an activation code at the end of each UR Medicine visit, so you can sign up at home when it’s convenient for you.

When you activate your MyChart account, you will immediately be able to send secure messages to your primary care clinician in Internal Medicine, Student Health, and Obstetrics & Gynecology. If you have proxy access for your child, you will be able to message their primary care clinician in Pediatrics. Please note proxy access for children is not available for children aged 13-17.
Results of common lab tests are immediately available in MyChart. Other tests are released 3 or 7 days after finalized , including imaging tests and pathology. Many providers will manually release them earlier, often with explanatory comments.
If you are admitted to a hospital in the Yale New Haven Health system, you will be able to view your schedule of medications and testing that take will take place throughout your admission. Once you are discharged, you will be able to view your date of admission and discharge along with your discharge instructions.
Or after your visit you will receive an After Visit Summary. The last item on the sheet is the MyChart information with your activation code.
You will generally receive an answer within a few business days. Please note that MyChart should not be used for urgent situations. Please contact your clinician if the situation requires immediate attention or dial 911 if it is an emergency.
MyChart gives you convenient and secure online access to your healthcare record. Whether you're at home, traveling or working long hours, you can access your medical history, communicate with your doctor, see lab results, set up appointments and more.
In addition to the MyChart mobile app, you can view your MyChart health information through the Apple Health app. If you have not already, register for MyChart. Open the pre-loaded Health app on your iPhone. In Health Data, go to the Health Records, find and connect to Wellstar Health System.
Once you’re connected, you can view your test results, schedule appointments, request prescription refills and pay your bills. You can also access your health information from organizations outside of Wellstar that also use Epic’s software, letting you have full control of your healthcare.

Can I view a family member’s health record in MyChart? Yes, you may view a child or adult's health record in MyChart.
As a MyChart user, your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.
For other than general information viewing, MyChart must be accessed using a secure browser. The latest versions of Internet Explorer, Firefox or Chrome are recommended. Along with user authentication, MyChart employs industry standard encryption protocols to protect your information.
To Subscribe to Receive Text Messages: To subscribe to receive text messages from Texas Health, log into your Texas Health MyChart account. Step 1: Go to Your Account in MyChart.TexasHealth.Org. Step 2: Click Profile-->Notifications to change your communication preferences.
To share your information, establish an account with the 3rd party application and review the application’s Terms & Conditions. Follow the instructions to find and select Texas Health. At the Texas Health MyChart log-in screen, enter your Texas Health MyChart username and password. Review the information that will be shared with the application which may include allergies, conditions, medications, lab tests, family history, goals, immunizations, procedures, smoking status, vital signs, and documents. If you consent, click Allow Access and continue to follow the steps provided by the 3rd party application.
An individual must be age 15 or older to request a personal account. Parents, guardians or other parental proxies of minor children may request access to the minor child's account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. We cannot offer the full spectrum of MyChart services to proxies of minor patients. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 15 or older may not view without consent of the minor patient. Because of these requirements, we cannot offer the full spectrum of services to parents of minor patients in this age category. Thus, when a minor patient reaches age 15, restrictions will be placed on MyChart access until he/she reaches age 18.

In some cases, the clinic staff that needs to respond to an electronic inquiry or other communication may not be immediately available, so a patient should allow at least three (3) business days for a response.
