What is MyMy St Francis?

My St. Francis is one complete report of your health care history allowing physicians throughout the St. Francis system to review your personal health records. With your permission, they can view your health history and collaborate on your care more effectively. View and share your personal health record related to your hospital stays.

What is the purpose of the MyCare service?

The purpose of offering you the MyCare service is to provide our patients with Web-based access to relevant portions of their medical records and to allow providers and staff to interact with patients regarding their health care and related information. The following terms and conditions ("MyCare Terms") govern your use of MyCare.

What is the MyHealth portal?

The MyHealth patient portal provides you, our valued patients, online access to your health information, whenever you need it. Note: You must be 18 years of age or older to access MyHealth. Disclaimer: DO NOT use MyHealth for urgent medical matters. If you are experiencing an urgent medical problem, call 911 or your physicians office immediately.

Why is MyCare not available?

You acknowledge and understand that MyCare may not be available to you all of the time due to system failures, back-up procedures, maintenance, or other causes beyond the control of the Service Provider. Access is provided on an "as-is, as-available" basis and the Service Provider does not guarantee that you will be able to access MyCare at any particular time.

What is my care service?

The purpose of offering you the MyCare service is to provide our patients with Web-based access to relevant portions of their medical records and to allow providers and staff to interact with patients regarding their health care and related information. The following terms and conditions ("MyCare Terms") govern your use of MyCare. Please read the MyCare Terms carefully, as they impose legal obligations on you and establish your legal relationship with Trinity Health Of New England and/or with your health care provider's office, if that provider is using MyCare. By accepting the MyCare Terms, you acknowledge that you are requesting access to portions of your health information via the Internet using an electronic application called MyCare. If you do not wish to be bound by the MyCare Terms, please do not register for the MyCare Service.

Where is MyCare governed?

The MyCare Terms shall be governed in all respects by the laws of the State of Connecticut without giving effect to its conflicts of law provisions. You agree to submit exclusively to the personal jurisdiction of and venue in the state and federal courts of the State of Connecticut, in the judicial district of Hartford.

Does Trinity Health of New England monitor physician records?

Trinity Health Of New England and its affiliates have no control over a physician's or laboratory's medical record entries, progress notes, or diagnostic or treatment recording, and does not monitor physician or other provider communications or entries of any kind via the MyCare service.

The MyCare service is not a substitute for telephonic or in-person communication with your health care providers and MyCare services should not be relied upon for medical decision making or as an alternative to office visits.
