What is a physician portal?

The patient needs to give a signed Authorization for Proxy Access to myBayCare form to a BayCare Clinic receptionist and provide proof of their identity. If you have Legal Authority to make care decisions for another adult patient, you will need to provide a copy of that document to BayCare Clinic in person in lieu of a signed Proxy form.
