MyUnityPoint can be used for many convenient day-to- day actions with your clinic and providers.

How long does it take to sign up for UnityPoint?

Anyone 18 years of age or older and is a patient of UnityPoint Health can register to use MyUnityPoint. It just takes five minutes to sign up, and your account will start within 1 to 3 days.

How to send a secure message to a patient?

Secure patient messaging allows you to: 1 Request a medicine refill 2 Ask a question about your lab results 3 Ask a question about your symptoms, treatment plan or medicine 4 Request an appointment 5 Provide information your care team has asked for (such as blood pressure, weight or blood sugar level) 6 Ask any non-urgent questions or concerns 7 Provide information or ask questions about your child’s health (with proxy access)

How to Register for MyUnityPoint

If you don't already have access to our patient portal, register for MyUnityPoint by visiting chart.myunitypoint.org. Request an activation code and it will be sent to your email. It may take up to a week for you to receive the code. If you need assistance, call MyUnityPoint Support at (877) 224-4430 or email MyUnityPointSupport@unitypoint.org .
