This Terms of Use Agreement (" Agreement ") is a legal agreement between you and University Health Network (" UHN ") which is the owner and operator of the myUHN Patient Portal website (" Portal "). This Agreement outlines the terms and conditions under which you may access and use the Portal and all written and other materials displayed or made available through the Portal such as videos, audio clips, articles, pictures, illustrations, links to third party sites, etc. (" Content "). By using the Portal, you accept to be bound by the terms and conditions of this Agreement. If you do not accept these terms and conditions, you must not use the Portal. UHN may revise this Agreement at any time and will update you changes via email as soon as possible. Using the Portal after UHN makes changes to the terms means that you agree to and accept the revised terms.
The Portal does not create a relationship between you and a health care provider. It is a tool to provide you with your health data as soon as it becomes available.
Choose a password that you do not use for any other system. The password must have a minimum of 8 characters, contain at least one (1) uppercase letter, one (1) lowercase letter, and one (1) number, and does not have user’s first or last name in it.
