Patient Portals are online health management tools that offer patients 24/7, convenient access to their personal health information, improve communication between patients and providers and streamline processes.

New Cerner Patient Portal

Our upgraded EHR solution utilizes one platform for all patient records. This means that whether you are seen in our ER, have a wellness visit at a clinic or come in for an x-ray, all of your information is housed in one system which you will access through one patient portal.

Past Patient Portals

Before our new Cerner System, patients had two separate portals to access information from the hospital and clinics. If you were previously signed up on our past portals, access them through the links below. Health information for visits moving forward will be in the new Cerner portal.

My Health Records and My Care Connection are patient portals that offer patients personalized and secure online access to portions of their medical records. It enables you to securely view information about your health. With My Health Records and My Care Connection, you can view:

Questions about your medical record

Your test results will be viewable in the My Health Records or My Care Connection patient portals immediately after the results are put into your health record. The following test results are currently not available to view: tests indicating sexually transmitted diseases, HIV, hepatitis serologies, pregnancy test and tumor markers.

Accessing medical records for your family

With MyHealthRecord, you can now access the hospital medical records of a loved one in order to help manage their care. My Care Connection does not yet offer the ability to see your loved ones records.

Technical questions

Click the "Forgot password" link on the sign-in page to reset your password. You will be prompted to answer the security question that you set up when you signed up for your account. If you need further assistance, contact toll-free My Health Customer Support for technical support, including password resets.

Accessing your medical records with health management apps

Health Management mobile apps give you the ability to view some of your medical records (or someone else's records, where you have proxy rights) on a smartphone or tablet.

You or your loved ones have the following responsibilities: 1 Providing honest and complete information about your current condition and about past medical conditions and treatments 2 Letting us know if you do not understand the description of your condition or the course of treatment proposed for your condition 3 Acknowledging that if you refuse treatment, you are responsible for the results of the decision 4 Following the treatment plan recommended by your health care team, including physicians, nurses and therapists. This responsibility also includes keeping appointments and giving notice when unable to do so 5 Paying or ensuring payment for any medical treatment received at Ascension Borgess, regardless of insurance 6 Respecting the rights of other patients and for Ascension Borgess personnel and property. This responsibility particularly recognizes that other patients may be harmed by noisy conversation or behavior 7 Following Ascension Borgess rules and regulations regarding patient care and conduct.
