To get to your patient portal, you’ll need to contact your medical provider’s office for the web address. They can also provide you with instructions on how to register and log in if you need them. How do I access the patient portal for Concord hospitals?

If you're a patient at Concord Hospital-Laconia or Concord Hospital - Franklin, and you want to access the patient portal for these two hospitals, visit concordhospital-laconia.org. Access test results, discharge instructions and visit summaries.

How do I enroll in my patient connect?

If you're at least 18-years-old and a Concord Hospital or Concord Hospital Medical Group patient you can request to enroll in My Patient Connect. Once you receive your invitation in your email account, you can register online.

Who can request access to their spouse's health information?

Legal guardians may request access to their ward's health information. Husbands and wives may request access to their spouse's health information. Adults may request access to their elderly parent's health information — with the patient's permission of course. Request Proxy Access.

What is clinical note?

Clinical Notes, the newest My Patient Connect feature, provides access to important information about your office visits and hospitalizations. The information available via Clinical Notes can help you remember your treatment plan and partner with your providers to ensure you have the best outcomes.

How long does a patient access link last?

Don't forget to complete your account creation soon. The patient-specific access links in the email message will expire after 90 days for your security.

How to retrieve my password?

Retrieving your password can be performed online. When attempting to log in, simply click on "Forgot Password.". Type in your email address and the words displayed for verification . Upon successful verification, you will receive an email containing a link and a few steps to reset the password.

How long does a link to enroll in a program last?

Simply click on the link provided in the email to verify your personal information. To protect your privacy, the access link will expire in 90 days.

Can you give access to your health information?

Only you can have access to your health information. You may grant permission to a caregiver or family member to also view your information. In the future, you will be able to securely transmit your health information to other health providers.
