MyHealth One Patient Portal Launch Recently Methodist Healthcare upgraded its patient portal to provide even more information for our patients and their caregivers. MYHealthOne is a secure online source for accessing health care information. The information is available online 24 hours a day from a desktop computer, laptop, table or smartphone.
To access the My OneCare Clinic Patient Portal please click here. This will include all physician practices and ambulatory facilities, e.g; UofLHealth - Primary Care Associates, UofL Health - Medical Center Northeast, etc. For frequently asked questions please click here.

If you are the parent or permanent legal guardian of a patient that is a child between the ages of 0-11 years old or a legal guardian of an adult patient or a person who has a Power of Attorney for healthcare over an adult patient, you can request proxy access.
1) Can I register for the portal online?#N#Yes! To self-register for a FollowMyHealth portal account, go to https://colnhc.followmyhealth.com, click “Create an Account”, choose your login method, fill out account information, click ‘Connect’ to PrimaryOne Health, then ‘Next’ to accept the terms and conditions.
