Do you have a patient portal?

My Health Story My Health Story is Meridian’s patient portal and electronic health record (HER) system. Clients can access My Health Story any time to schedule appointments, request prescription refills, access forms and manage their care. The information provided by Meridian is protected by Federal Law (HIPAA & CFR42).

Provider Portal

If you are a contracted provider, you can register now. If you are a non-contracted provider, you will be able to register after you submit your first claim. After creating an account within the MeridianHealth provider portal you can:

How long does it take to refill a prescription?

Clients may also call Medication Concerns at (352) 374-5600, option 3. Refills of medications are handled during normal office hours and can take up to 48 hours to process. Refill requests are not automatically approved and may require an appointment before refilling the prescription.

How long does it take to complete a disability form?

Please allow 10 to 14 business days for the forms to be completed.
