Once you are inside the portal, you will need to request your medical records. To do that click on ‘My Chart’ and then click on ‘Request Health Record’. Make sure you select ‘The South Bend Clinic LLP’ as your practice and if it gives you the option to select patient, make sure you select the correct person. Hit ‘Submit’.

Questions about your medical record

Your test results will be viewable in the My Health Records or My Care Connection patient portals immediately after the results are put into your health record. The following test results are currently not available to view: tests indicating sexually transmitted diseases, HIV, hepatitis serologies, pregnancy test and tumor markers.

Accessing medical records for your family

With MyHealthRecord, you can now access the hospital medical records of a loved one in order to help manage their care. My Care Connection does not yet offer the ability to see your loved ones records.

Ask your doctor how long you can expect to wait for test results. While many routine lab results could be available on MyBeacon within a couple of days, other test results may take several days or even weeks to become available on the portal.

Please send the patient name, date of birth and information you are requesting on your fax cover sheet. We fax the records to you unless there is a large volume of documentation. Those files are mailed.
