The Student Health Center will not share information concerning the health of a student without the student's explicit and informed written authorization.

Health Care Proxy

The Health Care Proxy allows you to name someone to make decisions about your medical care—including decisions about life support—if you can no longer speak for yourself. This form is encouraged but not required.

How do I get my immunization records sent to the Health Center?

New York State requires students to provide immunization records by the start of classes. We must receive your immunization records before classes start to avoid de-registration of classes.
