What is patient portal and how can it help?

Patient Portal You are about to access some of your personal health information from DCH Health System. Although we are making this information available to you electronically, the reports may contain information and language that is difficult to understand and could even create some confusion about its possible clinical significance.

Do you have a patient portal?

Go to the MyDCH portal site and click on the "Sign Up" link beside "Don't have an account?" You will be asked to confirm your identity for the Portal to access your records. You will need to enter your Last Name, First Name, Date of Birth, Last 4 digits of your Social Security Number and the email address you provided.
