MyChart Activation This tip sheet describes how to activate your MyChart account. 1. If you have had an appointment scheduled with WellStar, an activation code may have been automatically sent to your email address on file. This email will detail the steps on how to activate your MyChart account and will include a link with your activation code.
If you have an e-mail address on file then your MyChart Username will be sent to your e-mail account. If you do not remember any of this information, or you do not have a valid e-mail address on file, you will have to contact your MyChart help desk at 1-844-2NOVANT (266-8268) to help you regain access to your MyChart account.
If you forget your password, you will need to verify your username, date of birth and zip code. If you do not know any of the information below or you are having trouble recovering your password, contact the MyChart helpdesk (503) 494-5252.
You’ll receive an email containing a link that automatically enrolls you in MyChart. Click the link and follow the instructions to activate and access your MyChart account.
MyChart helps you securely manage and receive information about your health.
Access to a child's MyChart account automatically expires when the child turns 12. If you obtained proxy access to the child's account when they were between 12 and 17 years old, that proxy access expires again when the child turns 18.

Can I view a family member’s health record in MyChart? Yes, you may view a child or adult's health record in MyChart.
To access MyChart, you must create an account with your healthcare organization. To sign up for an account, download the app and search for your healthcare organization or go to your healthcare organization’s MyChart website.

MyChart is a free service offered by participating Inova medical facilities and doctors' offices that offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage your care and receive information about your health.
The free MyChart mobile application is available for iPhone and Android devices.
You can give someone else access to your MyChart. This can be done through the “Share My Record” option under “Sharing.” Once you’re on the “Share My Record” screen, select the “Friends and family access” option. Note: You’ll need to provide the email address of the person you’d like to have proxy access to your chart.

To remove a proxy's access, click “My Family Access” under the "Profile" menu, select the individual that you would like to remove, and click “Revoke Access”. If you do not have your own MyChart account, please call the Department of Data Integrity at 412-330-5399 and ask for your proxy to be discontinued.
G. MyChart unavailability. There may be times when MyChart is temporarily unavailable due to technical complications or during routine maintenance activities. AHN will make reasonable efforts to limit system maintenance to off-peak hours and resolve any technical issues as quickly as possible to minimize service interruptions.
