Midway Medical is now able to give our patients access to a secure portal with the capability to; request appointments, view lab results, change demographic information, and send emails directly to our office. New Patients

To inquire about becoming a new patient at Midway Medical Center, please print and complete the following Prospective New Patient Form. You may return your form to either office location in person, by fax, or by mail. Once your form is received, we will check physician availability for a new patient and our office staff will contact you.

Accessing Your Student Account

To access your student account, login to the MyMidway Portal and then select Self Service. You will be able to see your account balance, course schedule and other important information.
