Mount Carmel Medial Group has transitioned to a new patient portal, called MyChart, to better meet your needs. Any new visit information from October 9, 2021 and beyond, including prescription refills, visit notes and lab results, will now be in your new MyChart patient portal. Patients who wish to schedule elective medical and/or maternity services are required to pay a deposit and to make arrangements for payment of the balance of charges prior to receiving such services. A patient financial representative will contact you to arrange payment for your bill. If a balance remains after services are provided, a bill for the amount due will be mailed to you.
You will need to obtain a claim number from your employer. Mount Carmel must include this number on the bill that is sent to the insurance company. If you do not have a claim number from your employer, you are responsible for the entire cost of the services you receive.
You are expected to pay co-payments required by your health insurance plan at the time of service. Mount Carmel will bill your primary insurance company for you. If you have secondary insurance coverage, Mount Carmel will bill that company after your primary insurance benefits are processed. As a patient, you assume responsibility ...
Your bill from Mount Carmel is only for hospital services. Depending upon the services you received, you may be billed by your personal physician, other physicians who were involved in your care, and/or physicians who administered/interpreted your test results, and/or provided emergency care, radiology, pathology and/or anesthesiology services.
The authorization form is required when a patient or his/her representative is authorizing disclosure to another person/entity. You may download the “ Authorization for Use & Disclosure ” Form. Mount Carmel will accept an outside authorization form as long as it meets all legal requirements for a valid authorization.
Hours of operation are 8:00 a.m. – 4:15 p.m., Monda y - Friday, and we are closed weekends and holidays.

Each Heart & Vascular Center offers a wide variety of specialists and services all in one location – from cardiothoracic and vascular surgeons to cardiologists and heart and vascular imaging.
In addition to our on-campus locations, we have nine smaller centers throughout central Ohio to provide even more options and convenience. Some even offer cardiac and vascular rehab.
