Parent or guardian consent is required for most routine medical care in Michigan for students 18 years or younger. If your student will be 17 or younger when arriving on campus, please complete and return the Minor Consent Form.

Where is Moderna clinic at MSU?

Vaccine clinics are located in the MSU SUB on the lower floor between the Allen Yarnell Center for Student Success and Bobcat Grill. Currently, vaccine clinics are scheduled Mondays and Wednesdays from 10:30 a.m. to 1:30 p.m., and on some Fridays from noon to 3 p.m.
