Who can sign up for MyChart by Prime Healthcare?

Any patient over the age of 18 can sign up for their own MyChart account. A parent or guardian can request Proxy access for a child. Proxy Access request can be made by emailing mychart@primehealthcare.com

How to get activation code for MyChart?

Log on to MyChart, click sign up now and request an activation code. (Please note that online requests can take up to a week to process)
