Send the completed Authorization for Release of Information Form via mail to the address listed above, via email to MRPG-medicalrecords@milreg.org, or via fax to 508-478-0694.
Medical records are always mailed from the Medical Records Department. We do not fax or email records.
You can contact your child’s practice if you have additional questions about signing your child up for Patient Gateway.
But they may have unique privacy concerns. Because of this, the law treats the sharing of medical information differently for teens than for children. Patient Gateway’s access rules follow these laws.
If your child is age 12 or younger, you may access Patient Gateway by filling out the Patient Care Representative (PCR) Access Authorization Form. If your child is age 13 or older, both you and your teen must sign up together.

Through Patient Gateway, you can: 1 Check lab and imaging results, as well as letters and other reports from your providers 2 Access your health information from a non-Mass General Brigham institution if they are using the same electronic health record vendor (known as Epic) 3 Send non-urgent messages to your provider’s office 4 Request and view appointments, or make your own appointments (available at select offices) 5 Receive summaries of your visits 6 View, update or renew medications 7 Pay your medical bills online 8 Browse a library of trusted health information 9 Update your contact information 10 Complete questionnaires prior to your next visit 11 If applicable, enroll as a proxy representative when caring for a loved one or minor 12 Go green and conserve paper
As a parent or legal guardian, your account will automatically cancel when your teen turns 18. A parent, teen or physician’s office can ask for the Patient Gateway Account to be cancelled at any time.
