A patient portal is a website for your personal health care. The online tool helps you to keep track of your health care provider visits, test results, billing, prescriptions, and so on.

How do I register for MyChart at Mount Sinai Hospital?

Mount Sinai Hospital patients can register for MyChart in a few different areas in the hospital. You’ll need to provide your email address to get a PIN for self-registration. You will also need to have your OHIP card with you. Over the phone (416-586-4800 ext. 2649) - We will ask you a series of questions to validate your identity.

How do I register as a new patient?

Patients can also register at the Health Records Department Monday to Friday between 8:30 a.m. and 4:30 p.m. in two ways: In-person (2 nd floor, Room 213, 600 University Ave) Over the phone (416-586-4800 ext. 2649) - We will ask you a series of questions to validate your identity.
