Where is Mount Carmel Health System?

Mount Carmel is a member of Trinity Health, one of the nation’s largest Catholic health systems. Our new patient portal is integrated with other member hospitals and providers from across the country. Mount Carmel changed its electronic health record system in October 2021.

What is Mount Carmel Health System?

Mount Carmel Medial Group has transitioned to a new patient portal, called MyChart, to better meet your needs. Any new visit information from October 9, 2021 and beyond, including prescription refills, visit notes and lab results, will now be in your new MyChart patient portal. How to sign up for myDoctor's Office

If you're not already enrolled, request access at your next office visit. Our office staff will ask if you prefer to receive a text or email invitation.

How to Enroll via Invitation Email

Open the myDoctor's Office portal email invitation. Click the link to create your account.

Before your visit

Choose a private, well-lit, and quiet place for your visit. Make sure you have a comfortable place to sit for the visit and that your camera provides a clear view of you for the provider.

Check your technology

For a video visit you will need a camera-equipped phone, tablet or computer.

During your video appointment

Avoid streaming (such as music or TV shows) or, if possible, using other Wi-Fi dependent devices.

What to do if you have COVID-19 Symptoms

If you have a fever and or symptoms of upper respiratory illness, including dry cough or shortness of breath or have been in close contact with a person known to have COVID-19, please call your primary care office. Your symptoms will be reviewed by a provider who will recommend the next steps of your treatment.

Your Next Appointment

We all know how critical regular appointments are and we’re here to provide you the right care option, when you need it most. Before your next appointment or when scheduling your appointment, our office will discuss how/where you will receive your care (in-person, over the phone, or via a video appointment).

Co-Payments

In order to keep costs down, we expect payment at the time of service. Co-payments will be collected when you arrive, prior to your visit with the physician. We accept cash, credit or debit cards, and checks. If you have a balance, payment must be made within 30 days unless arrangements have been made with the Office Manager in advance.
