Accepted Plans

We accept the following insurance plans. If your insurance is not listed, please contact our business office at 358-3027 to determine if we participate with that plan.

Co-payments

Your co-payment is determined by your individual insurance plan. You will be expected to pay your co-payment at the time of service. If you receive a bill after paying your co-pay, your insurance has processed the claim and deemed that you owe an additional amount for the services you received in our office.

Billing Policies

Claims are usually processed by your insurance company within two to four weeks after your visit. If a balance remains due after our claim has been processed, we will bill you for that amount. Bills for a balance $25 or more are mailed on a monthly basis. Bills for balances under $25 are mailed on a quarterly basis.

Red Flag Policy

We are now required by law to request photo identification if you have not been seen in the last six months. This new policy is to protect you from identity theft. If your current address does not match the address on your identification, please bring a utility bill to verify your current address.

Notification of New Insurance

You should bring your child’s insurance card to each visit so that we can verify coverage. If your coverage has changed, we will ask to copy the insurance card for our files.

Physicians

Dr. Schweiker was born in San Angelo, Texas and grew up in multiple places, including Rome, Italy. Her family eventually ended up in Lynchburg, Virginia where she graduated from E.C. Glass High School.

Nurses

Susan joined MAP in 2003, and enjoys working with the Doctors in a supportive team atmosphere. She loves helping new parents to adjust. Susan and her husband Chris have 3 children and 5 grandchildren as well as Kiki her tea cup Chihuahua and her newest additions grand dogs Evee and Mina.

Office Staff

A native Richmonder, Tarnetta is very active in her church. She enjoys reading, walking, and singing in church choir. She especially enjoys the outreach ministry where she works with the homeless and the elderly. She loves spending quality family time with her husband, children and grandchildren.
