How do I sign up for your patient portal?

Scheduling. Once the Scheduling department has an order from your physician, an appointment can be scheduled for you. Patients with lab orders or general x-ray orders can walk-in anytime between 6:00 am and 10:00 pm. To schedule an appointment, call (812) 825-0818.
