When you give another person proxy to your medical records in the Portal , this person can see the same records that you see in the Portal. The proxy has the same viewing rights as you do.

Mission Community Hospital (MCH) is a two-campus acute care hospital located in the San Fernando Valley. Licensed for 85 medical/surgical beds and 60 acute psychiatric beds, we provide a full range of medical, surgical, psychiatric and emergency care. We are accredited by the Joint Commission as a Gold Seal of Quality Service hospital. And, we are licensed to participate fully in the Medicare and Medi-Cal programs. In addition, we are proud to have been designated as a California Disproportionate Share Hospital, with 94% of our services being provided to Medicare and/or Medi-Cal beneficiaries.
Mission City Community Network serves the low-income and uninsured populations in the San Fernando Valley primarily in the surrounding communities of North Hills, Van Nuys, Panorama City, Pacoima, Mission Hills, and Reseda. MCCN also provide service to patients throughout the San Fernando Valley and in Hollywood.
MCCN received it’s Federally Qualified Health Center status in 2007 in recognition of the high quality comprehensive care that it provides to the growing number of uninsured children and adults in the community. Mission City Community Network is committed to providing quality healthcare services to those most in need.
MCCN participates as a healthcare provider in several programs for low-income individuals and families. Former Medi-CAL recipients and other low-income persons are encouraged to inquire about these special programs.
Mission City Community Network (MCCN), a not-for-profit organization with multiple clinics operating as a Federally Qualified Health Center dedicated to serving low income and under-served families in the San Fernando Valley, San Gabriel Valley and Los Angeles and San Bernardino areas.
Responsible for the outreach component of the various programs; for outreach into the community, via various avenues; to assist with the health education and health promotion of certain programs and to provide exposure of specific programs and Mission City Clinic in general.
