How do I self-enroll in Mid-Valley Hospital&Clinic?

For information prior to October 14, 2019, please use Previous Patient Portals. Self-enrollment is available for Mid-Valley Hospital & Clinic patients who are 13 years or older. Click HERE to self-enroll! If you manage the health care of another person, for example your child, provide your email address to a member of our health care team.

Does miDmiD-Valley use Cerner health?

Mid-Valley uses Cerner Health to give you access to your own medical record, connect with our health care team and more! You may also access information for family members and individuals for whom you provide care for. (When granted access) The NEW Health Patient Portal only includes medical information that has been added after October 14, 2019.

What is the Alliance spine and Pain guest network?

This guest network is provided for the use of visitors to Alliance Spine and Pain Centers and is subject to monitoring by the IT department. There is no expectation of privacy when using this network.

New Patient Forms

Before you come into our office on your first visit, we ask that you would please download and print out these forms. Please fill them out and bring them with you. This will help speed up the process, and more importantly, give you more time in the comfort of your own home to fill them out accurately and legibly.

Previously Treated Chiropractic Patients

We cannot begin to tell you how many of our patients come to us who are previously treated Chiropractic patients from other Doctors and have seen little to no success.
