Will my patient records be transferred to Middle Tennessee family medicine?

We are excited to have Warren Langworthy, M.D., Scott Eller, M.D. and their associates at Middle Tennessee Family Medicine (MTFM) join our MMC family! To read the full letter from Drs. Langworthy and Eller, click here. As of July 1, 2021, all patient records, including yours, will be transferred to MMC.

How do I make sure I am receiving patient portal emails?

To ensure that you are receiving Patient Portal emails please do the following: Add NextMD.com to your contact list, address list, safe list, or "Do Not Block" list. Account will be locked for 20 minutes after 4 failed login attempts. Please remember that password is case sensitive.

How to contact FMH for proxy account?

If you are interested in a proxy account for your child’s records, please contact FMH Support at 888-670-9775 (an account will not be processed if you use the link below for minors between the ages of birth - 17).

This FREE self-service health management tool can improve interactions with your doctor’s office, document important health care information and ultimately give you the ability to make more informed decisions about your health!#N#If you are interested in a proxy account for your child’s records, please contact FMH Support at 888-670-9775 (an account will not be processed if you use the link below for minors between the ages of birth - 17).#N#• Please note that parents can obtain access to their child’s records from birth to age 12, but the MMC and Me account will be suspended when the child turns 13 years of age due to TN state law that allows the minor to obtain certain treatment without parental consent. New accounts will not be created for the ages of 13-17 due to this law. All requests must be made by a legal parent only.

When will MMC transfer medical records?

Medical Records. As of July 1, 2021, all patient records, including yours, will be transferred to MMC. MMC will maintain all medical records according to the same standards of confidentiality as MTFM and as required by the Tennessee Board of Medical Examiners.
