One thing to note about this demo is that, with the exception of PatientID autonumber keys are not used, but 'natural keys'. No you don't need want separate tables per patient. That's a maintenance nightmare.
If you need to get a history of all Meds for a patient, it would be easier if the patient was identified in the Meds record rather than having to go through the Visits table. This is one place where I might consider a little denormalization by including both Patient and Visit IDs as foreign keys in the Meds table.
