Cancellation Policy

If you are not able to keep your appointment, please notify our office. We appreciate a 24 hour notice, if possible. There is a $25.00 no-show fee charged to patients who do not provide notice of cancellation.

Bill Payment

Payment of co-pays and non-covered services are due at the time of service. You will receive a statement from us showing your balance, after insurance payments, on a monthly basis. Payment is due within 30 days after the billing date. For your convenience, we accept personal checks, Visa, MasterCard, Discover, and American Express credit cards.

During Your Visit Instructions

No children are allowed during your visit, except for infants accompanying during 6-8 week post-partum visit. Absolutely no strollers. Patient will be asked to reschedule. Thank you!

We work hard to accommodate the needs of all our patients. Please bring your insurance information, photo ID, completed paperwork ( see below ), or arrive 1/2 hour early to complete your paperwork in the office before your appointment.
