What is a complete physical?

A complete physical is defined to include a physical examination, discussion of risk factors for cardiovascular disease and cancer, with focus on a healthy lifestyle that includes diet and exercise. It also includes review of any recent tests and recommendations of needed screening tests and/or immunizations.

Do siblings need to have their own appointments?

If you are scheduling well children appointments for siblings, please note that each child needs to have their own individual appointments . This ensures we will have adequate time to address your concerns about each child.

Does Metro Family Physicians use Sharp Lab?

Metro Family Physicians Medical Group utilizes both Sharp Laboratory and Quest Diagnostics. For those patients who have an HMO you must use Sharp Lab for your laboratory services.

How old do you have to be to communicate with FPC?

Parents/guardians may communicate with FPC providers and staff as you always have until your child reaches age 18.

Can a parent/guardian be a portal authorized representative?

For this reason, we find it necessary to ask our patients in this age group to complete a consent form naming a parent/guardian as their portal authorized representative. Without this consent from the minor patient, we will not be able to give parents/guardians access to the minor's portal. (Click the link below to access the form.)

South Branch Family Practice is the family care practice of Julie Ann Juliano, M.D. and Ruby Halper-Erkkila, M.D., both experienced family health practitioners . The focus of our practice is to provide your family with complete healthcare services. We offer routine and preventative care services including: physical exams and immunizations, illness and injury treatment, chronic disease management and specialist referrals.

Is South Branch Family Practice covered by insurance?

Emergency Note: There is Always a doctor on call for South Branch Family Practice. Please call the office first, BEFORE you go to any urgent care center. These centers are not covered by most insurances and you will have to pay out of pocket.
