How do I access information from the previous patient portal?

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.

Merit Health Madison provides CareNotify ™, a system to help you be informed about your care and what you need to do to stay healthy. CareNotify ™ includes a Patient Portal as well as capabilities for direct communication to patients via text or email.

How to grant access to MyHealthHome?

To grant a caregiver, guardian or trusted family member / friend access to your MyHealthHome account, the requestor will first need to create their own MyHealthHome account. Once they have created their own account, they can request access to your account. You then will receive notice of the request along with an auto-generated PIN code to share with the requesting individual. Once the requesting individual has entered the PIN code, the accounts will be linked and the requestor will be granted access to view your health information.
