To enroll, simply give your valid e-mail to the Registrar when you register for your hospital visit. Once you have logged into MyHealth, a separate link on the left navigation bar can take you to the login screen for MyHealth@Mercy to easily access records at your provider’s office.

Children under 12 years of age and dependent adults who have a Dignity Health doctor may have a My Care (Patient Portal) account with an adult as the proxy. Accounts are not available for adolescents 12 – 17 years of age. For a dependent adult, they will need to sign a release, or you will need to provide a copy of the Medical Durable Power of Attorney before an account can be created for the dependent adult.

The enrollment link expires 90 days from the day they are issued. If you are not able to find your invitation or it expired, you may request a new invitation from the staff at the facility that assist with your medical records. If you need technical help to locate the invitation, assistance enrolling, or navigating within the portal, you may contact our Technical Support center at (877) 621-8014; the staff are available 24 hours a day, seven days a week.
Please note -- include the patient’s signature and date of signature, located at the bottom of the form. If the patient is unable to sign, please provide the appropriate legal documentation needed to prevent any delays in retrieving records.
