Appointment Cancellation Information

If an appointment has to be canceled, please inform our office at least 24 hours in advance. When an emergency or illness arises, we will do our best to reschedule your visit as soon as possible.

Waiting Time Information

We strive to ensure that each patient is seen as close to their appointment time as possible. Occasionally, emergencies do arise. If this is the case, you will be informed promptly of any delay to your appointment time.

Materials to Bring to your Appointment

Please remember to bring any X-rays, MRI's, CT Scan's or EMG reports that pertain to your injury as well as a list of current medication and dosage information. Please have any other relevant office notes or operative reports from another physician's office sent to us before your scheduled appointment.
