A portal can be established for any age, however, for patients between the ages of 12-18, a proxy agreement letter must be signed, and a proxy assigned if a parent/guardian would like access to the portal. Please email portal@swhealth.org for more information.
If you did not provide an email address during a past visit, you can contact us a portal@swhealth.org and request to be registered for the portal (s). Please be sure to specify in your email whether you want access to the Hospital Portal, Clinic Portal, or both.
An email address is required to create an account for either patient portal. For the hospital portal, multiple family members can use the same email address, however the clinic portal requires a separate email address for each user. If you do not have one, you may create an email account free of charge from various sources online.
Only one parent/guardian can accompany the patient to the appointment. (Unvaccinated Visitors are Permitted) One designated parent/guardian (s) is allowed at all times within the facility. And second parent/caregiver is allowed between the hours of 4:30pm through 8:00pm everyday, for all admitted patients.
Universal Mask Visitor requirements 1 Visitors: All visitors will be provided a surgical mask by the hospital. Visitors may wear it over any personal mask. Masks must be worn continuously until they leave the building, per Universal Mask Procedure. 2 Gaiters or masks with uncovered valves are not permitted.
No food or liquid beverages may be brought into Behavioral Health at this time. All visitors and patients must remain in the assigned area/room during visitation. They will be screened and must follow universal mask requirements and visiting guidelines. No visitation within the psychiatric emergency department.

A patient and consumer health portal that saves patients time and offers the convenience of accessing their health information from Delta Health 24-7 is now available. Patients will need to have a valid email address on file at the hospital in order to access the patient portal.
