What is a patient portal?

The Memorial Health patient portals are a secure, easy way to view your medical information as a patient. It is a free, online, patient-managed personal health record. Download clinical data, including lab and X-ray results. Change demographic information (address, phone number). Export your data to a flash drive.

Via MyChart, you can communicate with your doctor, access test results, request prescription refills, and manage your appointments. We offer MyChart to patients at the following hospitals:

For patients at St. Luke’s Health–Brazosport Hospital, we offer a separate, secure portal that allows you to access your health information, update your personal information, check account balances, and view scheduled appointments all in one place.

Patients Medical Center Patient Portal (MEDITECH)

For patients at St. Luke’s Health–Patients Medical Center, we offer access to MEDITECH. Log in to your account to find your medical records, schedule a visit, and more.

Memorial Hospitals Patient Portal (MY ONECARE)

For patients at St. Luke’s Health–Memorial, we offer access to MY ONECARE. Log in to your account to find your medical records, schedule a visit, and more.

Recommend an App

You can use another application to access your health information, if you prefer. To do so, you will need to first set up a patient portal account. The username and password you choose will be the security component for your health information and make it possible for you to access and import it.
