The Portals makes time-consuming tasks simple...a few clicks, and you are done. Patient Portals allow you to access your health information online versus over the phone or in person.

What is Memorial Hospital address?

Patient Portal. The Memorial Health patient portals are a secure, easy way to view your medical information as a patient. It is a free, online, patient-managed personal health record. Download clinical data, including lab and X-ray results. Change demographic information (address, phone number). Export your data to a flash drive.

You will need your personal medical record number, your birth date, and an e-mail address. Visit my.huntsvillememorial.com and follow directions to set up an account.

Once Enrolled

Patients may access their personal electronic medical record at any time and from anywhere through the Huntsville Memorial Hospital website.

Important Notes

Patient Portal records only include medical record summary, test results, and medications given at Huntsville Memorial Hospital. List of medications only includes those electronically entered and/or prescribed at patient's discharge. Patient Portal records are for the patient's information only.

How to Register to use Your Patient Portal

To register and gain access to your patient portal, you must provide an email address to the hospital and clinic receptionist at your next appointment.
